Effectiveness of a Khorasan Wheat-Based Replacement on Pain Symptoms and Quality of Life in Patients with Fibromyalgia

Objectives:
This study evaluated the effects of a replacement diet with Khorasan wheat products in patients with fibromyalgia, comparing them with a similar diet using control products made from organic semi-whole modern wheat.
Methods:
A randomized, double-blind, crossover trial was conducted with 20 participants (19 women and 1 man, mean age 48.9 ± 12.3 years) diagnosed with fibromyalgia. Participants were randomly assigned to consume Khorasan wheat products (pasta, bread, crackers, biscuits) or control products for eight weeks, after which the groups were switched. At the beginning and end of each intervention period, validated self-administered questionnaires were collected from each participant.
Results:
Statistical analysis using a general linear model for repeated measurements, adjusted for potential confounders, showed overall improvements in questionnaire scores after both intervention periods, with more significant effects after the Khorasan wheat period. Notably, a 21.5% reduction in the Widespread Pain Index (WPI) and Severity Scale (SS), as well as an 11.7% improvement in the Functional Outcome of Sleep Questionnaire (FOSQ), were observed only after the Khorasan wheat period. No significant changes were reported after the control period. Similarly, scores on the Fibromyalgia Impact Questionnaire (FIQ) decreased significantly by 22.5% after the Khorasan period compared to only 0.3% during the control period (P=0.037). The improvements were greater in participants with more severe symptoms.
Conclusions:
Dietary intervention with Khorasan wheat products appears to provide benefits for patients with fibromyalgia, particularly those with more severe symptoms.
